Attention Cooperative/Dispensary you must register to verify patients even if you are verifying by phone.

Please click on read more for instructions.

1. Click on register below the login on the left.
2. Select Cooperative/Dispensary Profile.
3. Make sure to fill out all the information including the managers FIRST name and business phone number.
4. MAKE SURE TO ALSO PUT THE MAIN BUSINESS EMAIL ADDRESS not personal employee emails they will be deleted.
5. After you register you must check your email and click on the link to verify your email address.
